Terms and conditions

I, Serena Davis (the Practitioner) trading as Health by Serena, provide nutritional therapy, health coaching, private and group yoga classes, and pregnancy yoga classes. I intend my classes and my advice to be accessible, helpful and of benefit to you (the Client). Because we live in a commercial world where services are provided and paid for, we need to have terms and conditions so that you have a clear understanding of where we stand.

 

Terms and conditions

 

  1. Payment: All fees for classes and appointments must be paid in full at least 24 hours prior to the start of each session.
  2. Cancellations: The Client must provide at least 24 hours' notice for cancellations or rescheduling of appointments. The Client is required to pay for sessions that are missed or cancelled with less than 24hrs notice. A 100% cancellation charge applies if appointments are cancelled with less than 24hrs notice. Non-attendance of an appointment will also be charged in full. If the Client does need to cancel an appointment, please let the Practitioner know at the earliest convenience so we can reschedule the appointment. This is done by emailing hello@healthbyserena.com. If a cancellation occurs that is instigated by the Practitioner, and less than 24hrs notice is provided, a 50% fee reduction for the next appointment will apply. It is the Client’s responsibility to have a working internet connection for consultations via Zoom and telephone. Full consultation charges will apply for scheduled sessions that are unable to connect.
  3. The Client understands and agrees to the following. I am responsible for contacting my GP about any health concerns. If I am receiving treatment from my GP or any other medical provider, I should tell him or her about any nutritional strategy provided by my nutritional therapist. If I am not clear about the agreed nutritional therapy program/supplement doses/time period I should contact the Practitioner promptly for clarification. I understand that the advice is personal to me and might not be appropriate for others. I must contact the Practitioner should I wish to continue any specified supplement program for longer than the original agreed period to avoid any potential adverse reactions. If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact the Practitioner promptly for clarification. I must tell the Practitioner about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme. I give permission for the Practitioner to contact my GP regarding any agreed aspects of my case.
  4. Health and Medical History: The Client is required to provide accurate and up-to-date information about their health and medical history. This information will be used to tailor the yoga classes, nutritional therapy, and health coaching to the individual needs of the Client. The Client understands that the completed health questionnaire will be used as a basis for the first consultation or yoga class.
  5. The Practitioner will not be liable for any adverse effects resulting from the Client’s failure to disclose relevant medical information, or to adhere to the agreed-upon nutrition and lifestyle plan.
  6. The Practitioner cannot be held liable for any injury, loss or damage incurred during classes or sessions, or because of the services provided.
  7. Confidentiality: The Practitioner will maintain strict confidentiality regarding all information shared by the client during sessions, in accordance with legal and ethical standards.
  8. Recording of consultations and classes by the Client using any form of electronic media is not allowed.
  9. Anyone under the age of 18 will need to be accompanied by an adult.
  10. Scope of practice- Nutritional Therapy is the application of nutrition science in the promotion of health. Nutritional therapy practitioners use a range of tools to assess and identify potential nutritional imbalances and understand how they may contribute to an individual’s symptoms and health concerns. This approach allows us to address nutritional balance and help support the body towards maintaining health. Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, and those looking for support to enhance their health and wellbeing. Practitioners consider everyone to be unique and recommend personalised nutrition and lifestyle programmes for each client. We would never recommend nutritional therapy as a replacement for medical advice and always refer any client with ‘red flag’ signs or symptoms to a medical professional. We can work alongside medical professionals and communicate with other healthcare professionals involved in a client’s care to explain any nutritional therapy programme that has been provided.
